CORI Checks & Policy
This form is to be used by the State Grange Junior and Youth Departments, and Junior Grange leaders.
These forms are required for any individual who will be working with minors in an capacity on a regular basis during any Grange sponsored event. All individuals who are working with minors must complete the form and provide a copy of a government-issued photo ID, such as a driver’s license or passport. Completed forms and copies of IDs must be mailed to the State Secretary for processing.

Payment Vouchers
This form is to be used by State Grange officers, deputies and committee members.
This form must be forward to the State Master, along with copies of receipts, in order to receive reimbursement for approved purchases.

Activity Request Forms
All State Grange departments and Junior Granges must submit this form for all events offered to the general Grange membership or to the public in order for the event to be covered by the State Grange’s insurance policy. Forms must be submitted to the State Master at least four weeks prior to the event.
